Client Intake Forms
Please download and complete the proper intake form prior to your first session. Return to me by email or bring with you when you come in. Alternately, you can fill it out when you arrive.

Getting There
Location
Soul Alchemy LLC - 111 S First St, suite 190, Madison, WI 53704
Parking
There is parking in a small lot just north of the building. (To the left when facing front of building, and just across the railroad tracks.) Alternately, street parking is available on Atwood Ave.
Building Access
Main Entrance: Enter at the front door on 111 S First St and look for suite 190. (Turn left upon entering the building, and go all the way down to the end of the hall on the right.)
Accessible Entrance: Park in the designated spot in the small lot off the Atwood side of the building. (Look for 1912 Atwood Ave and park in it's attached lot.) Enter through the courtyard and choose the door marked "entrance to offices". Go down the hall and turn right. My office is all the way at the end and on the right.
(Avoid the entrance at 1912 Atwood Ave, which only accesses the residential apartments, not the offices. )
Arrival
Please arrive at your scheduled time, or within 5-10 minutes previous to your scheduled time, not earlier
In Case of Illness
Please do not come to your session if you are ill or have any of the following symptoms: fever, sore throat, cough, stuffy nose, respiratory symptoms, or other cold or flu like symptoms.
